The following items are necessary prior to enrolling your child in kindergarten through fourth grade. 

We may require additional documentation or information. Please contact our school  for further details.


Age Verification:

• Original or certified copy of birth certificate (Mandatory for first time Kindergarten; if possible, for grades 1st-8th)

Immunization Records:

• Up-to-date immunization records including a Mantoux type TB test and required shots for age level

Physical Exam:

• Physical Exam for 1st grade only

Information from Previous School:

• Check-out papers from previous school (not required but extremely helpful)
 
• Most recent IEP if student is in Special Education classes
